New Patient Packet

Your Exam
We’ll reserve an hour for your first visit so that we have plenty of time to get to know you. After completing your new patient forms, we’ll discuss your goals and expectations and the reason for your visit. You’ll meet Dr. Ramsey, who will perform a comprehensive exam of your teeth and gums and check for signs of decay and periodontal disease. He’ll also inspect your existing dental work for wear and may suggest replacing metal fillings and crowns with mercury-free, tooth-colored restorations. An oral cancer exam is a standard part of every new patient exam.

To help you become an informed partner in your dental care, Dr. Ramsey will take pictures inside your mouth with an intraoral camera and display them on a chairside monitor. He’ll use these images to explain his findings and suggested treatment options.

A thorough cleaning to make your smile sparkle can be scheduled prior to your exam, after your exam, or at a separate appointment. Tell us your preference when you call to schedule your new patient visit.

Payment Options
For your convenience, we accept cash, checks, and all major credit cards. AARP members receive a 10% discount on all services. View our Special Offers page for more information. Insurance Information
We accept all dental insurance plans and will electronically file your claim for you. As the patient, you are responsible for all applicable co-pays and deductibles.
